GENERAL PURPOSE OF THE JOB
This position is responsible for the delivery of all nursing services for Chronic Kidney Disease patients
under their care.
SUPERVISORY RESPONSIBILITIES: No
ESSENTIAL DUTIES AND RESPONSIBILITIES
The following duties and responsibilities generally reflect the expectations of this position but are not
intended to be all inclusive:
• Performs direct patient care, determined by the area of practice including but not limited to
taking:
o Monitoring vital signs, physical assessment of patient o
Transcribing physician orders
o Drawing and documenting ordered laboratory work o
Documenting treatment and observations
o Administering medications according to physician orders o
Providing dialysis therapy (HD/PD)
o Providing vascular access (VA) care and assessment for HD o
Assisting with VA center procedures
o Providing appropriate information to the patient regarding their care
• Analyzes patient assessments, laboratory data and, when concerns are noted, brings issues
to the attention of physician
• Communicates all emergencies, patient life-threatening complications and related incidents to
the Head Nurse, Nephrologist, Director of Clinical Services and/or Regional Operations Director
• Assesses patient and family learning readiness and provides appropriate information
• Maintains a high standard of clinical practice and safe working environment
• Promotes and maintains safe infection prevention and control practices
• Instructs patients regarding dialysis, access care and chronic kidney disease (CKD)
• Instructs patients on pre-transplant preparation
• Administers patient quality care in a considerate, respectful manner
• Participates in long- and short-term care planning and monthly progress notes
• Attends mandatory training as part of the onboarding process
• Assumes responsibility for administrative/clerical duties as needed
