BACKGROUND:
Of the 7.6 million deaths in children under five years of age that occurred in 2010, over 40% were during the first 28 days of life. The risk of mortality is greatest during the first week after birth, during which about 70% of neonatal deaths occur. Many of these newborn deaths can be prevented by simple interventions delivered at the community level.
Evidence from several research studies suggests that home visits by community health workers or volunteers (CHW; CHV) during pregnancy and in the first week after birth can make a significant difference in reducing newborn mortality. At these home visits in these studies, CHWs/CHVs counselled the families on home care practices during pregnancy and after birth and encouraged families to seek appropriate care during pregnancy, childbirth and the postnatal period.
In 2016 UNHCR began strengthening support to neonatal care in both Azraq and Zaatri camps. A baseline assessment was conducted in February and March in these two camps which are home to close to 100,000 refugees. A significant gap in care uncovered in this assessment was the lack of routine postnatal check-ups for the newborn after discharge. In all facilities, it was found that routine postnatal visits were done on the mother only, and HIS indicators are recording mother-only visits. In both camps, CHVs (there are no CHWs in Jordan) conduct home visits, but their work with antenatal women and newborns is under development and needs to be strengthened through increased training, coordination, and job aids. As a result it was agreed with other organizations working in community health to strengthen this component of community health programming in both camps.
CHVs will learn to conduct home visits to pregnant women to promote antenatal care, planning for skilled care at birth, and home care during pregnancy. They will also visit newborns and mothers in the home in the hours and days following birth (with a focus on days 1, 3 and 7) identify danger signs and refer appropriately, and advise on appropriate home care practices for newborns and mothers.
SCOPE OF WORK:
A consultant is sought to develop a training package to teach CHVs to care for mothers and newborns in the community, to assess for danger signs, and to assist families in accessing clinical care when necessary. This will be done in collaboration with organizations working in the field, including the Ministry of Health, IRC, Medair, UNHCR, UNFPA, UNICEF, and JICA. The work shall be from approximately July 10 to October 13 2016.
