As a member of the multidisciplinary team, the hospice nurse works under the general direction of the patient care coordinator and the hospice physician in the provision of skilled nursing assessment, planning and care to patients and families at home.
RESPONSIBILITIES :
The Hospice home care Nurse is responsible primarily for identifying patient and family needs. The nurse is responsible for providing supportive compassionate care in accordance with the hospice philosophy, policies and procedures, and physicians orders. The nurse, while coordinating input with the patient / family, coordinates the resources of the interdisciplinary team to develop an individual care plan.
FUNCTIONS:
1- Initiates / maintains communications with attending physicians, other hospice staff members and other hospice members and other agencies as need to coordinate optimal care and use of resources for the patient / family.
2- Maintains regular communication with the patient.
3- Provides the following:
a. Patient / family emotional support.
b. Educates the patient / family regarding medications, nutrition and symptom management.
c. Provides family bereavement follow-up support.
4- Reports family needs to the volunteer coordinator for the provision of appropriate volunteer services.
5- Uses physical assessment to determine ongoing nursing needs as well as pain assessment skills to maintain good symptom control.
6- Maintains records-keeping in compliance with hospice format as stated in hospice policies / procedures manual.
7- Attends interdisciplinary team meeting, admission conferences, staff meetings and inservice workshops.
8- Maintains close communication with hospice physician, the patient services coordinator and hospice team members.
9- Is responsible for supervision of contracted nurses, *associate nurses, home health aides, nursing students and the hospice volunteer.
10- Participates in service / continuing education related to palliative care.
11- Coordinates physical care of patient by teaching the primary family caregiver.
12- Assumes shared 24-hour, seven -day-a-week, on call coverage to patients / families.
13- Assumes role of the *associate nurse when responding to patients / family needs when taking on call or when requested by the patient care coordinator in the absence of the other primary nurses.
14- Provides and keeps a current nursing care plan to implement and provide coordinated.
15- Serves on peer record review committees.
16- Complete incident reports as specified in policies and procedures manual.
17- Maintains, in confidence, all patient / family records.
18- Supervises medication administration as ordered by the physician and educates family members in safe medication administration. Disposes of all controlled medicines in accordance with hospice drug policies.
19- Participates in hospice orientation and in-service training programs for hospice professional staff.
