Job brief
We are looking for a Case Manager / 2 years of experience in GBV / NGOs to enhance the quality of client management, The successful candidate will be accountable for the full case management. Case Manager duties include assessing, planning, implementing, monitoring, and evaluating actions required to meet the client’s care plan, protection services, and human services needed.
Responsibilities
- Fulfills beneficiaries ' requirements by completing intake interviews, determining the need for medical, psycho-social, and psychiatric evaluations in addition to the development of case objectives.
- Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered
- Handle case assignments, planning, and follow_up of care plans,
- review case progress and determine case closure
- Facilitate multiple care aspects (case coordination, information sharing, etc)
- Prepares reports by collecting, analyzing, and compiling statistics in addition to completing grant and cash assistance
- Help beneficiaries make informed decisions
- Record cases information, complete accurately all necessary forms, and produce statistical reports
